Balanitis Xerotica Obliterans BXO is a chronic, often progressive disease, which can lead to phimosis and urethral stenosis, affecting both urinary and sexual function. Steroid creams are usually the first-line treatment but have a limited role and surgical intervention is frequently necessary. Conservative surgical procedures circumcision are often preferred in the first instance with the premise that recurrence of disease will require a more definitive reconstruction. The aim was to look at their management in the past and subsequent management by us.

Diagnosis: Penile lichen sclerosus balanitis xerotica obliterans When lichen sclerosus LS , a chronic, progressive, sclerosing inflammatory dermatosis, involves the penis, it is termed balanitis xerotica obliterans BXO. BXO manifests as ivory, white, or purple-white patches or plaques on the glans and prepuce of the penis and sometimes on the frenulum, urethral meatus, fossa navicularis, penile shaft, and anus. Lesion consistency can range from sclerotic to atrophic. Surface texture can range from keloidlike to cigarette-paperlike. Scar tissue is hard and can prevent retraction of the foreskin.
